Fibromyalgia Syndrome: Taking Another Look

Throughout modern medical history, fibromyalgia has been seen as a unique pain disorder of the musculoskeletal system, a disorder of abnormal sleep processing, a centralized disorder of pain processing, and a psychosomatic illness. Now researchers are proposing possibly one of a few genetically similar central pain syndromes—high impact chronic musculoskeletal pain conditions or overlapping pain syndromes. Over the past 5 to 10 years there has been much focus and research on the fibromyalgia syndrome. Diagnosis is much better understood now than ever before, allowing for research into best outcomes in treatment and a new understanding of the role of pharmaceutical therapeutics. This presentation will focus on what frontline clinicians need to know about the current understanding of fibromyalgia pathophysiology, criteria to making the correct diagnosis, and evidence-based recommendations for pharmacological and nonpharmacological options for treatment.

Risk for Opioid Overdose:
Identifying At-Risk Patients in Your Practice Hidden in Plain Sight

What is the profile of someone at risk for overdose? For many, the perception of overdose risk is tied to someone who is an injection drug user, someone with an opioid use disorder that takes more than they can handle, or someone who unknowingly encounters a drug laced with dangerous levels of fentanyl.1 Few would dispute that these individuals should have ready access to the opioid reversal agent naloxone. While these are accurate profiles of individuals at risk, is the profile too narrow and biased with misconceptions?

The 2016 CDC Guideline for Prescribing Opioids for Chronic Pain recommends co-prescribing naloxone for patients at risk for overdose, including patients with a history of overdose or substance use, patients taking benzodiazepines, and patients taking ≥ 50 MMEs of opioids daily. Yet, in a March 24, 2020 CME Outfitters Risk Evaluation and Mitigations Strategies (REMS) webcast, among 477 internists, primary care providers, nurse practitioners, and PAs, 73% co-prescribed naloxone in patients receiving high dose opioids (≥ 50 MME/day) less than 50% of the time. Thirty seven percent never co-prescribed naloxone in this patient population. The objective of this virtual symposium is to raise awareness about the risk of overdose among patients with chronic pain in everyday practice and actionable changes to practice that are needed to mitigate that risk.

The Other Opioid Crisis: Heroin and Fentanyl

There is a significant amount of media, political, and public attention paid to the opioid crisis/opioid epidemic in the United States today. With the seemingly ever-increasing number of opioid-related overdoses and fatalities, there has been a feverish push by stakeholders to diminish the amount of opioids prescribed in order to help stem these worrisome trends. Unfortunately, there may be a lack of focus regarding the true definition and characterization of the opioid epidemic. There may also be a rush to judgment about the role of appropriately prescribed opioid analgesics in the addiction crisis we face today as well. This presentation will discuss the roles and statistics of both prescription and illicit opioids (namely heroin and fentanyl) in today’s “opioid overdose epidemic” with the intention of clarifying important differences and similarities between these competing epidemics including concerns and clinical considerations specific to each of them. Additionally, this program will examine and identify how these medications and drugs share potentially tragic adverse effect profiles in many cases. However, it is important for clinicians to make sure that appropriate chronic pain patients that may be candidates for opioid analgesic therapy aren’t penalized, and still get the treatment that they deserve.
