Kratom or Bait'Em Revisited
Faculty Jeffrey Fudin

As clinicians and their patients scramble for opioid anal¬gesic alternatives and ways to blunt opioid withdrawal, kratom (also known as mitragynine) use among desper¬ate patients has surged. Thousands are self-treating, and clinicians are left to carry the bag of benefits vs risks of this natural substance as a single agent and combined as part of a polypharmaceutical regimen. Added to this dilemma is whether or not to include kratom with routine toxicology monitoring. Concerns around health and safety have spurred USregulatory agencies to scrutinize attributes and detrimental effects, variations among products, quality control, and adverse health outcomes. This session will focus on the history of kratom, epidemiology, advocacy among sup¬port groups, product inconsistency among manufactur¬ers and related dangers, pharmacology, therapeutic risks vs benefits, and dangerous drug-drug, drug-disease interactions.

Falling Down the Rabbit Hole:
A Primer for Chronic Pain Management and Substance Abuse Disorders
Faculty David Cosio

The field of pain management has undergone a circuitous adventure, much like a rabbit hole. As the economic, mental health, and medical consequences of prescribing opioid medications have mounted, the prevailing logic regarding the usefulness of prescribing opioids for chronic pain has shifted. The widespread dissemination of opiates and the lax safety measures placed on their storage has also led to an increase in nonmedical use. Given the high level of comorbidity between opioid use disorders and chronic pain, providers’ decisions about how to address treatment with patients who may have or who have been diagnosed with substance use disorders are often complex. The new CDC guidelines will require providers to assess for risk of overdose or development of a substance use disorder, and to be keenly aware of their patients’ pain levels and pain management strategies when working as part of a system where opioid medications may be prescribed. Participants will learn how patient and provider education programs and communication interventions may improve outcomes in pain management. Participants will also learn how to select candidates for opioid trials, assess for risk, and initiate opioid therapy, but only after exploring nonopioid and nonpharmacological strategies.

CE/CME and Accreditation

This activity is provided by Global Education Group.

Target Audience

The educational design of this activity addresses the needs of frontline clinicians: physicians, nurses, nurse practitioners, physician assistants, and pharmacists involved in managing acute and chronic pain.

Statement of Need/Program Overview

Whether you're in a hospital or office-based practice, chronic pain patients are everywhere--and YOU are absorbing the burden of their care.

A just released report by the National Institutes of Health outlines positive steps that practitioners can take in response to the burgeoning problem of prescription medication abuse and to provide more effective pain management care to their patients. Key among these is access to pain education, and by participating in a PAINWeekEnd Conference, you'll improve your skills in medication risk evaluation and mitigation, in pain assessment and diagnosis, and in the delivery of individualized multimodal treatment.

The PAINWeekEnd agenda is purposely constructed for the busy clinician, delivering a full day of relevant, practical information, together with the opportunity for interaction and exchange with faculty and fellow attendees.

Pain Management for the Main Street Practitioner has been created to streamline the clutter of information on assessment, evolving guidelines, risk management, and changing reimbursement scenarios. Join us for a day of clinical and practice management CE/CME designed expressly for frontline clinicians. Participants can receive 3.0 AMA PRA Category 1 Credits™ as they expand their capabilities in the assessment, diagnosis, and management of chronic pain conditions.

Learning Objectives

  • Identify regulatory concerns, quality control issues, and variability among kratom dosage forms
  • Describe the high level of comorbidity between opioid use disorders and chronic pain
  • Define rational polypharmacy and when it is indicated for pain management
